The Volunteer Clearance Form must be submitted by all parents/guardians each year to participate in school functions.
The Field Trip Transportation Form in Private Vehicles must be submitted by all parents/guardians each year in order to drive students to/from field trips in their private cars. A copy of the insurance coverage declaration must be included.
*Volunteer Clearance approval also required to transport students.
The Field Trip Authorization Emergency Medical Information Form must be submitted by all parents/guardians each year to allow students to attend field trips.
